Major fly problem. Dozens of flies throughout kitchen. Bagged garbage laying at back door outside standing water also attracting flies to kitchen. New delivery food items stacked on ground next to garbage waiting to be loaded in to walk in. Injecting BBQ with unapproved insect/pest pump sprayer with injector needle to apply apple juice in to meats. Use only approved equipment for food contact surfaces and food handling. Store raw hamburger meats beneath whole cuts of meat, pork and beef.

Intent to suspend issued for 2603 a grade card shall be posted in a conspicuous place readily observed by public upon entering. Grade card removed for second time.
